Do you take insurance?
Due to today's medical climate and the complexity of medical insurance, most physicians are forced to have brief appointments with high patient volume. Because I believe in individualized personal care I do not participate in any insurance plans, Medicare or Medicaid. Once seen you will be given a detailed receipt which can be submitted to your insurance plan by you for reimbursement. All payments are due at the time of appointment. This model has allowed me to provide a higher quality of care and form a stronger relationship with my patients.
What are your payment methods?
Dr.Parks' accepts personal checks, all major credit cards and cash for payment of services. Payment is required upon receipt of services.
How should I prepare for my upcoming appointment?
Please complete the intake information by completing the attached forms under the “patient forms” section and bringing them to the office with you. Please bring in all current medication bottles. Please obtain your mental health records from your most recent provider and bring to the appointment or fax to the office prior at 732-447-2554. Please remember to bring your driver's license and your insurance card with you.
What if I need to change an appointment?
If you cannot keep your appointment, please call 732-447-2545 to cancel or reschedule your appointment at least 48 hours in advance. Failure to do so will result in cancellation fees equal to that of a missed appointment. Missed appointments will be charged at the full appointment rate. When an appointment is made with Dr.Parks, time is specifically set aside for you. When an appointment is missed without notice, another patient who needed to be seen may have been unable to do so because that time slot was already filled.

48 hour cancellation policy and fee: If you cannot keep your appointment, please call 732-447-2545 to cancel or reschedule your appointment at least 48 hours in advance. Failure to do so will result in cancellation fees equal to that of a missed appointment. Cancellations and rescheduling can be made via phone, text or email.
Missed appointment policy: Missed appointments will be charged at the full appointment fee. When an appointment is made with Dr.Parks, time is specifically set aside for you. When an appointment is missed without notice, another patient who needed to be seen may have been unable to do so because that time slot was already filled.
Missed appointments and medication refills: If an appointment is missed enough refill medication will be called into your pharmacy to last you until your next scheduled appointment. Please note that 30 or 90 day supplies will not be called in without appropriate in office follow up care. This may mean that only 2 weeks of medication may be called in which may result in higher medication payment at the pharmacy.
Appointment payment: Payment is due at the time of service. Missed appointment fees are due prior to the next scheduled appointment or are to be paid in full at the next scheduled appointment if arrangements cannot be made for payment to be done in advance.
Medication Refills: Please allow at least 3 business days for refill prescriptions to be called into your pharmacy. Please note that the office is closed Friday afternoon through Sunday. After hours refill requests should contact the after hours office line.
Letters and Forms: Please allow for at least 7 business days for forms and letters to be completed. There is no guarantee that these can be completed prior to this allotted amount of time. A fee may apply for filling out of forms and an office visit may need to be scheduled for longer forms.
Insurance Receipts: Please keep all insurance receipts for your records and for submission to your insurance carrier. If insurance receipts are lost, replacement copies cannot be guaranteed.

We are happy to offer virtual telemedicine sessions via doxy.me. These sessions are available upon request for those patients who are unable to make it into the office for appointments. Please note, all initial intake sessions must be done in person but virtual sessions are available thereafter.
