Due to today’s medical climate and the complexity of medical insurance, most physicians are forced to have brief appointments with high patient volume. Because we believe in individualized personal care we do not participate in any insurance plans, Medicare or Medicaid. Once seen, you will be given a detailed receipt which you can submit to your insurance plan for reimbursement. All payments are due at the time of the appointment. This model has allowed us to provide a higher quality of care and form a stronger relationship with our patients.

The Parks Center accepts personal checks, all major credit cards, and cash for payment of services. Payment is required upon receipt of services.

Please complete the intake forms by clicking the link and bring the completed forms to the office with you. Please bring in all current medication bottles. Please obtain your mental health records from your most recent provider and bring them to the appointment or fax them to the office at 732-447-2554. Please remember to bring your driver’s license and your insurance card with you.

If you cannot keep your appointment, please call 732-447-2545 to cancel or reschedule your session at least 48 hours in advance. Failure to do so will result in cancellation fees equal to that of a missed appointment. Missed appointments will be charged at the full appointment rate. When an appointment is made with our providers, time is specifically set aside for you. When an appointment is missed without notice, another patient who needed to be seen may have been unable to do so because that time slot was already filled.

A D.O. is a doctor of osteopathy or osteopathic physician. It is a type of medical training that emphasizes practicing “whole body” medicine.
