Before your visit, please complete the “New Patient Appointment” forms emailed to you at the time of booking. We kindly ask that they be submitted at least 48 hours prior to your appointment. These forms give our providers helpful background so your time together can be as meaningful and productive as possible.

And just a gentle reminder of our 48-hour cancellation policy — appointments cancelled or rescheduled with less than 48 hours’ notice are subject to a $225 late cancellation/no-show fee.

If you're joining us virtually, simply log in about five minutes before your scheduled appointment time. You'll receive your secure telemedicine link in advance via email and text with your personalized appointment reminders. There’s nothing to download and no apps to install — just click the link from your phone, tablet, or computer, settle into a private space, and we’ll meet you there. Simple, seamless, and designed to fit into your life.
This is the same link used for all virtual visits, so feel free to save it for future appointments. We’ll give you a quick call shortly before your virtual appointment time to confirm payment and make sure you were able to access the link without any trouble.

We understand that life happens. If you need to cancel or reschedule your appointment, we kindly ask that you call us at 732-447-2545 at least 48 hours in advance.
Appointments changed with less than 48 hours’ notice, or missed without notice, are subject to the full session fee. When you reserve time with one of our providers, that space is thoughtfully set aside just for you. Providing advance notice allows us to offer that time to another patient who may be waiting for care.

Letters & Forms
Need paperwork completed? We’re happy to help. To give your request the attention it deserves, please allow about 7 business days for forms or letters to be completed. While we always try to move things along quickly, we can’t guarantee turnaround sooner than that.
A small $25 administrative fee may apply for standard form/letter completion. If a form/letter requires more detailed information, we may ask you to schedule a quick visit so we can make sure everything is completed thoughtfully and accurately.
Whenever possible, we recommend sending forms/letters a little ahead of time so we can take care of them without any last-minute stress. Thank you for your patience—we appreciate it!

Absolutely. We offer both in-office and virtual visits, giving you the flexibility to choose what feels most comfortable. Our providers are licensed in New Jersey and New York, allowing us to care for patients across both states.
